Come join us as we continue the tradition of having a joint meeting with the CT Valley AIHA.  Nathan Fague will lead the meeting as he gives a presentation on Root Cause Analysis.  The outline for the training is below.

  4. Easy ways to integrate RCA into your existing safety programs
    1. Accident reports/ investigation
    2. Near miss reports
    3. Upset business conditions and other problem-solving opportunities

Speaker Nathan Fague, MS

Nathan (Nate) Fague currently serves as the Director of Safety & Security at Mystic Aquarium, where he oversees safety, security, occupational health and environmental compliance for the campus community. With progressively greater responsibilities since joining Mystic Aquarium, Nate has served in functions related to safety and security for the past seven years.

With a Bachelor’s degree from the University of Connecticut in Occupational Safety & Health, a certificate in Environmental, Health & Safety Management from Three Rivers Community College in Norwich, CT, Nate also holds a Master’s degree in Safety & Occupational Health Applied Sciences from Keene State College. He is also a certified First Aid/CPR/AED instructor through the American Red Cross and sits on the Association of Zoos and Aquariums Safety Committee.

A Past-President and active member of the American Society of Safety Professionals (ASSP), Nate currently serves as the Communications and Scholarship Chair of ASSP’s Connecticut Valley Chapter. He is also a member of the Connecticut River Valley Chapter of the American Industrial Hygiene Association, Connecticut Safety Society and the Connecticut Biosafety Alliance. Nate regularly provides lectures and training opportunities centered on engaging zoo and aquarium staff to become more safety-conscious and to mitigate the unique occupational hazards encountered in these environments.
